If you are enrolling your child for the summer session, fill this form out.

This form gives us permission to use your child's picture on our website and in our advertising.

This form informs us who is allowed or not allowed to pick up your child after school.

If the instructors / therapists need to administer medication to your child during class, this form will need to be signed and kept on file.

Family handbook with key information form

The form at the back of the handbook needs to filled out completely for any new children that are enrolled. A new one of these will need to be filled out for any existing children should the information within the form change for any reason.
